Eligibility: All individuals and families with sickle cell disease and sickle cell trait in Philadelphia, Chester, Bucks, Montgomery and Delaware counties.

Intake Procedure: Client intake process is completed by requesting demographic information, type of sickle cell disease, special health conditions; hospital affiliation; income; emergency contact, etc.
Intake Requirements: Self and family referrals; medical centers, hospital/ agency social workers; community-based and social service agencies, Department of Human Services; legislative offices; schools, colleges universities; fraternities/sororities and immigrant organizations, etc.
