Hospice program enables patients with a serious, progressive illness to live at home as fully and as comfortably as possible wherever home may be, even a nursing home or personal care facility. The hospice team includes nurses, social workers, home health aides, a chaplain, on-call nurses, volunteers and a bereavement coordinator.
