Provides Medical Case Managers who work in partnership with clients to remove practical and emotional barriers to primary medical care, access to medications, housing, benefits and entitlements, and transportation services.

  • Members of the LGBTQ2+ elder community (age 55+)
  • Individuals living with HIV
  • Individuals with opioid use disorder who are incarcerated or recently released from incarceration.
Support also offered to clients' families and friends.
Hours: Office Hours: Monday through Friday, 9:00am to 5:00pm
Emergency services coverage offered 24/7.
