Smoking cessation program available for patients who show the desire to want to quit smoking.

Eligibility: Must live or work in Luzerne County. Must be employed, temporarily receiving unemployment benefits or actively looking for work. Must not have private insurance, or federally funded insurances such as Medicaid. Household income at or below 200% of the Federal Poverty guidelines.
Hours: Monday through Friday , 8:00am to 4:30pm

Intake Requirements: Patients must provide proof of identity, proof of work status and household income (recent pay stubs, a recent W-2 form, or an employer's verification of wage) and proof of residence, recent tax return.
