Assists adults with services to enhance the quality of life and to support physical and emotional independence. Case management services are designed to meet physical, social and material needs. Services are provided in a person's own home, in our office, or at other convenient locations. Provides assessments and benefits check up to assure appropriate information and referrals. Volunteer match may be available to help with basic concerns.
