Social Services: A member of our social services staff meets with each resident and conducts a social history and assessment upon admission. When short-term residents are ready to return home, social services staff helps residents determine the best options for a safe discharge. This includes reliance on home health care, durable medical equipment, community services, and support groups.

Social services staff assesses the psychosocial needs of each long-term resident and visits for socialization and emotional support. The social services staff also acts as a liaison for community services that might be required for residents. All residents are invited to care conferences facilitated by the social services staff where the residents progress and plan of care are discussed.
