Intensive Case Management /Resource Coordination/Family Based Mental Health Services
Provides non-clinical primary mental health services to insure that the most seriously mentally ill individuals receive additional support geared to improving the quality of their lives and reducing psychiatric hospitalization. Casemangers use natural occurring supports in the community, i.e., family, friends and neighbors. Services include: Psychiatric Evaluation, Medication Monitoring Service Coordination, Psychiatrist Supports Crisis Intervention, Social Skills Training Psychiatric Rehabilitation, Vocational Assessment Housing Supports, Activities of Daily Living Peer Support Services, Leisure Time Training
