Full service hospice program providing comprehensive physical care by nurses and home health aides; assistance with home, family or legal issues by a medical social worker; emotional and spiritual support from a spiritual care counselor; practical help from trained volunteers; and follow-up bereavement support for caregiver and family for a period of one year.

Eligibility: Anyone no longer seeking or receiving curative treatment for a life-limiting chronic illness or disease (includes cancer, endstage cardiac, lung, liver or kidney disease; Alzheimer's or other endstage dementia; ALS; MS - to name a few)
Referral can be made by attending physician, family member or friend.
Hours: Monday through Sunday, 8:00am to 5:00pm; 24/7 on-call service
