Intake Procedure: Application must be submitted by a healthcare professional who is a member of the person in need's treatment team. Individuals may not submit applications.
Intake Requirements: Only those applications submitted by a healthcare professional (social worker; nurse; etc.) who is a member of the applicant's treatment team will be considered. Patients and/or family members may not apply directly for funds. Criteria is NOT income based.
Geographical Area: Blair, Centre, Clearfield, Huntingdon, Juniata, and Mifflin Counties
