Intake Procedure: Appointment must be made at least three working days in advance. If you leave a message, please provide a full name, phone number, the date and time of appointment and the appointment location (Altoona or Pittsburgh).
Intake Requirements:
  • Must be able to get in and out of the vehicle on own
  • Can only transport portable oxygen tanks
  • Provide photo ID
  • If aide is needed, a letter from your primary care provider indicating that you require an aide to travel is needed. All other above requirements apply to the aide as well

Helpful Tips: More convenient pick-up points will be considered on a case-by-case basis. This is a shared ride system and pickup times vary based on individual appointment times. Drivers are authorized to cancel trips due to severe weather conditions
