Intake Procedure:
Intake Requirements:
a. Print out from the Food Stamp, Child Support, Copy of pay stubs for yourself and others in the house.
b. Print out for you social security, SSI, and/or Veterans Income.
c. Any other income such as direct deposit, retirement, unemployment, alimony
d. certified written statement from family or friends that help or have helped pay your bills.
2. All bills paid on a monthly basis with receipts showing what you paid.
3. Financial Crisis Documentation - Each case will be determined upon your emergency.
4. If you are seeking assistance for a utility bill, you must have the most recent 2 months bills. No assistance with disconnect fees or deposits. The bill must be due or past due.
5. Any other items requested by the Caseworker when scheduled appointment.
