LIFE is a nationally recognized Program of All-inclusive Care for the Elderly (PACE). LIFE provides medical, health and social services that promote independence at home, for as long as desirable and feasible. LIFE serves seniors with a range of disabilities including chronic physical illness, memory loss, mental illness or mental retardation that impacts activities of daily life. LIFEâs team of geriatric experts evaluates each LIFE participantsâ and their familyâs needs, goals and preferences to provide care that is custom-tailored.
