Intake Requirements:
- Photo ID (drivers license, state ID, passport)
- Proof of birth for each child (birth certificates, medical insurance card, state benefit letter)
- Proof of all Income (pay stubs for last 30 days, 2022 tax return, 2022 compass report, 2022 ssi award letter)
- Proof of address (utility bill or current lease)
- Toy suggestions written on paper is helpful to bring (no electronics, bikes, or large gift items)
- If applicant has foster or adoptive children, court documents are required
