Eligibility: Age 18 and older, currently in a state of withdrawal with at least 20 to 24 hours having passed since last use (certain exceptions may apply).

Must have been opioid-dependent for at least one year.

Fee Amounts: Accepts Medicaid, Medicare, commercial insurance, and self-pay.
Intake Procedure: Call for information.
Intake Requirements: 1) A valid form of photo identification that verifies the patient’s place of residence.

2) Proof of any prescription medications, if necessary.

3) Fee for the intake assessment and/or
insurance card, if the patient intends to use insurance to pay for services.
