Provides care coordination. Offers assessment, referral, recovery planning, care monitoring, medication monitoring, and interface with prescribing physicians and outpatient treatment programs to provide a team approach.

Eligibility: • Adults from age18 to age 65 who are engaged in opiate addiction or those already on Buprenorphine and Vivitrol
• Works with individuals on medical assistance, self-pay, or those eligible for county funding.
Hours: Monday through Friday, 8:00am to 4:00pm

Fee Amounts: No fee for individuals on medical assistance or is county funded. Self-pay clients will get fee schedule during phone intake.
Intake Procedure: Call first for a phone intake, then scheduled for a face-to-face appointment.
