HealthyCare Card (HCC) is a program of Healthy Community Network (HCN).
HCN is a collaboration with Health care providers and social service agencies who are committed to working together to improve access to healthcare services for those in our region who are uninsured or under-insured.
A person must enroll in health insurance if eligible for it. (For example: Medicare, Medicaid or commercial insurance through a job. A completed application with proof of income and assets will determine what discounts a person may be eligible to receive.
A person with the HCC and no other prescription coverage will be given a list of pharmacies where prescriptions can be dispensed at a discount. The discount on other care will depend on
