Provides supports to allows adults with disabilities to live purposeful and stable lives in the comfort of their own homes. Services include up to 30 hours a week of customized services and supports, independent living skills training, community engagement support, assistance with medical appointments, building connections, and assistance with choosing, acquiring and maintaining a job.
