Intake Procedure: Call 211 or 855-567-5342 to complete a CHART pre-screen Monday through Friday, 8:00am to 5:00pm.

Self-referrals, walk-in to Restart Day Center at 360 Locust Street or call 717-449-0492 to complete an intake/pre-screen and be placed on a processing list Monday through Friday, 8:00am to 3:00pm.
