Intake Procedure: All cases are served on a first come first serve basis. You can pre-register online, or request an appointment by calling 610-619-7370.
Intake Requirements: Please bring all of the following that apply when you come for intake: Picture ID, Insurance cards, Second proof of residency if uninsured, Hospital discharge instructions, Documents from any recent treatment, Documents relevant to any recent medical conditions, List of medications, Current and past treatment information, and Legal documents pertaining to treatment.
