Erie West Senior Center - Aging In-Home Services
Provides for the physical, social, & emotional needs of the senior citizens through informal and formal supports that enable seniors to make informed choices, to be safe, to remain independent, and to be involved in the community.

Care Management provides an in-depth evaluation of client needs and the facilitation of the most appropriate services and monitors those services and the person's individual service.

Services may include: Personal Care, Home Support, Meals on Wheels, Adult Day Care, Emergency Response System, Home Health, and/or Medical Supplies.

Eligibility: Age 60 or older.
Determined to be in need of the services in order to maintain independence.
Hours: Monday through Friday, 8:30am to 4:30pm

Fee Amounts: A small co-payment is required for some services.
Intake Procedure: A caseworker will identify the needs of a consumer utilizing the appropriate assessment tools. Unmet needs that can be addressed through In-Home Services will be part of an individual service plan developed with a care manager.
