Eligibility: Anyone age 18 and over. Minors under the age of 18 are not able to consent to vaccinations on their own. They must be accompanied by a parent or legal guardian to the vaccine clinic.

Intake Requirements: Must bring:
A) Face Mask
B) ID, drivers license or other government issued identification
C) Download, print, complete and bring the COVID-19 Vaccine Registration Form to appointment
D) Health Insurance Card
Geographical Area: Only vaccinating people who reside in the Penn Highlands primary service area.
