Meadville - Mental Health Blended Case Management
Supports individuals to gain access to needed resources such as medical, social, mental health and other related services. Involves frequent contact and provides emergency mental health crisis intervention. Based on individual strengths focused on improving natural supports, independent living skills and reducing the placements outside of the home. BCM does not replace any existing services, but helps locate, coordinate, and monitor services in place to assist people in their mental health recovery.
