Intake Procedure: Intakes are done on a walk-in basis
Monday through Thursday, 8:00am to 6:00pm
Friday, 8:00am to 2:00pm
Intake Requirements: Insurance and/or referral information if applicable. Personal ID. Works with all patients, regardless of insurance or ability to pay, to assure access to care.
