Housing Transitions
State College - Adult Services Case Management
The Adult Services Case Management program coordinates a variety of assistive services for individuals with a goal of maintaining independence and the ability to stay within their current home.

While the Adult Services Case Manager does not coordinate medical care, they coordinate many other services and is a great resource for clients.
