Hazelton - Heartwood Center
Provides support to individuals and families who are at risk of experiencing homelessness, housing instability, and/or food insecurity. Provides a day shelter providing an immediate and temporary place for individuals and families experiencing homelessness to stay during the day hours while they seek to regain permanent housing stability. The goal is to ensure that individuals are connected to supportive services and permanent housing opportunities.
Services include:
A} Case Management and Recovery Support Services
B) Life Skills Education and Computer Access
C) On-Site Food Pantry with daily breakfast and lunch
D) Employment Development
E) Housing Support and Transportation Assistance

Eligibility: Open to adults age 18 and older in Hazleton area facing homelessness.
Hours: Daily, 8:00am to 6:00pm
