The School Walk Kit provides school nurses, educators and volunteers with lesson plans and classroom activities that stress the importance of eating healthy and the value of physical fitness. American Diabetes Association - African American Programs
Diabetes is one of the most serious health problems that the African American community faces today. Compared to the general population, African Americans are disproportionately affected by diabetes. To address this growing epidemic, the American Diabetes Association created a targeted approach called African American Initiatives. The initiative is designed to increase awareness of the rates of diabetes among African Americans, provide information about the seriousness of diabetes and its complications, teach the importance of making healthy lifestyle choices, and educate those with or at risk for developing diabetes about prevention, treatment, and management. African American Initiatives develop culturally appropriate materials and community-based activities that empower, educate and create measurable differences in the prevalence of diabetes and its complications among people of African descent. The vision of the initiative is a healthier community living free from diabetes and it's complications. The target audience is all people of African descent living in the United States whether they identify themselves as being African American, Black, African, or from the Caribbean. Community-based educational programs and workshops are an important part of the African American Initiatives. Through fun and informative workshops conducted by trained Association staff and volunteers, community members can learn more about diabetes, the importance of making healthy food choices, and being physically active on a regular basis. Materials and events targeting the African American community are also an integral part of the Association’s outreach efforts to help improve the quality of life for African Americans and their families.

If threat of an outbreak that mass immunizations may prevent or limit, the center may quickly establish mass immunization sites among the non-immunized population. - High risk infant follow-up program for families without health insurance, who have premature infants, or infants with congenital defects. Home assessment, health education, and growth and development monitoring are provided by a nurse. - Information and referral services on Hepatitis C, AIDS, genetic disorders, cardiac, cleft palate, cystic fibrosis, phenylketonuria, spina bifida, sexually transmitted diseases. If there is a program at the state level for specific disorders, the staff will assist local residents in accessing these services. - PKU follow-up provided to infants with elevated phenylalanine levels. Nurse will act as a liaison between the family and the PKU Treatment Center. - Rabies prevention. All animal bites are reported to the State Health Center. Both victim and owner are contacted and informed of protocol for confinement of the animal. If there is a risk of rabies, a nurse will assess and educate the parties involved regarding the need for rabies prophylaxis. - Community Education Programs on such topics as: AIDS, diabetes, health risk education, high blood pressure, and nutrition. Farm and fire safety education programs for health fairs and parochial schools, including Amish schools. Health and safety educational programs for day care and HeadStart staffs and WIC parents. - Provides information on West Nile Virus.

    Programs that help people of all ages who have juvenile or adult diabetes understand and manage their illness. Services may include screening programs for early identification of the condition and associated complications; establishment of a treatment regimen; patient education; and self-management skills development assistance which may focus on use of insulin and drugs to control blood sugar levels, blood glucose monitoring, dietary management in different environments (at home, at work, in school, while dining out), menu planning, the role of exercise in relation to food intake, hygienic measures such as foot care and ways to reduce the likelihood of diabetes-related complications which may include heart disease, kidney failure, amputation, nerve damage and blindness. Diabetes is a chronic, incurable disease in which the pancreas fails to produce enough insulin to allow the body to adequately metabolize sugar.
