Community CARES Cumberland Street Reach
CUMBERLAND STREET REACH: Community CARES began a Street Outreach program during 2018. The goal of Street Outreach is to engage, assess, and combat homelessness for individuals living on the streets, in encampment and other locations not intended for human habitation. Outreach staff work to build trust with those experiencing homelessness first by helping to meet some of their basic needs (including food, clothing, showers, laundry, transportation, identification, etc.). The street reach coordinator works with each individual to access housing, employment, medical care, mental health care, substance abuse treatment, and other services while advocating for housing solutions for clients.

Eligibility: For Shelter: Single men/women over the age of 18, as well as families are eligible for services. All shelter guests were required to go through a National Criminal Investigation Check (N.C.I.C.) Visit the Carlisle Police Station at 240 Lincoln St, Carlisle to obtain NCIC clearance and bring that with you when you come to the shelter. Criminal sexual offenders who appear on the Megan's Law list are not admitted to the C.A.R.E.S program.

Covenant House Pennsylvania - Outreach
The Outreach Team provides Outreach services (food, drink, blankets, hats, gloves and scarves, counseling and referrals) to youth who are on the street or unable to come into shelter. They work to develop trusting relationships with youth in an effort to encourage them to utilize the services at the Crisis Center. The Outreach team collaborates with other agencies and organizations to help meet the youths’ immediate needs while on the street and can transport youth to Covenant House as well as other agencies.

We will offer screening services for the inner-city community, including substance-abuse addicts, the homeless, prostitutes, and youth, which are high-risk for HIV, diabetes, hypertension, and tuberculosis. We will increase access to health care by referring individuals to where they can receive further medical care, counseling, or support services. Our outreach workers are dedicated to follow-up on each patient.

Emergency Services and Crisis Intervention services provide JFK Mobile Emergency Team, Central Intake / Crisis Intervention, Crisis Specialist and Outreach to the Homeless. Crisis intervention at any location is available 24 hours a day, 7 days a week. An outgrowth of the JFK Mobile Emergency Team is the Crisis Specialist Program. Crisis Specialist professionals meet the consumer at his or her location, stay to stabilize the situation and determine if further care is needed. This service is also available through the same delegate line as the JFK Mobile Emergency Team. Central Intake / Crisis Intervention provides a central access point to help reduce the use of psychiatric hospitalization and connect people to services with the least disruption possible to their everyday lives. Outreach to the homeless is ongoing and takes place in various areas of the city as the need arises.

Clearinghouse Program: Provides rental assistance and other housing-related needs to mentally ill homeless, transitioning youth/adults (age 18-23), residents of a community rehabilitation residence and individuals being release from incarceration. Homeless Supportive Services: Provides intensive case management and referrals to other support services for single, adult, chronically homeless individuals. Assist street homeless men and women to connect to community resources as they strive to move from homelessness to self-sufficiency. Pharmaceutical Assistance Program: Provides uninsured Lehigh County residents with once-a-year emergency pharmaceutical assistance of up to $50. Must meet income guidelines. Assistance for medications for asthma, epileptic seizures, diabetes, high blood pressure and antibiotics only. Social Security Representative payee Program: Provides financial monitoring and fund administration to individuals receiving social security who are unable to manage their own finances. "

Outreach teams provide medical care, basic supplies to provide relief from cold weather, and help individuals access housing and other social services.

A day center that provides services to the homeless and homeless at risk in Monroe County. Some of their services include case management, linkage to mental health services, linkage to drug and alcohol services, medical clinic, dental clinic, and vocational support. The population served ranges from those in risk of homelessness to the actively homeless. Other basic services like bathroom and showering facilities, as well as laundry facilities, are also available. You must call for availability. There are no overnight services available with this program.
