Engage, assess, and combat homelessness for individuals living on the streets, in encampment and other locations not intended for human habitation.

Outreach staff work to build trust with those experiencing homelessness first by helping to meet some of their basic needs (including: advocacy, food, clothing, employment, showers, laundry, transportation, identification, an address to receive mail,etc.).

Operates 5 days a week. Outreach workers go out to neighborhoods, parks, and other places where youth who are facing homelessness seek refuge. Offers food, drink, blankets, hats, gloves and scarves, counseling, and referrals to youth who are on the street or unable to come into shelter.

G-PATH Housing Outreach Specialist will initiate face to face
contact and establish a working interaction with a homeless or
at risk of being homeless individual or family to
assist them with obtaining adequate shelter and facilitate access to additional services.

Provides a continuum of care for chronically mentally ill and substance abusing homeless populations. Includes shelter outreach, day therapeutic programming for the dually diagnosed, outpatient, and supported independent living. Programming is accessible through the homeless shelter system or through the Alternative Services Unit of the Behavioral Health.

A drop in center open overnight from 7pm until 6am, 365 days a year for homeless individuals to meet their immediate needs by providing a safe place to sleep, healthy food, showers and season-appropriate clothing. The center has a capacity of 50 individuals and 75 during code blue.

We will provide health care to meet the spiritual, physical, and emotional needs of the community. We will offer screening services for the inner-city community, including substance-abuse addicts, the homeless, prostitutes, and youth, which are high-risk for HIV, diabetes, hypertension, and tuberculosis. We will increase access to health care by referring individuals to where they can receive further medical care, counseling, or support services. Our outreach workers are dedicated to follow-up on each patient.

Emergency Services and Crisis Intervention services provide JFK Mobile Emergency Team, Central Intake / Crisis Intervention, Crisis Specialist and Outreach to the Homeless. Crisis intervention at any location is available 24 hours a day, 7 days a week. An outgrowth of the JFK Mobile Emergency Team is the Crisis Specialist Program. Crisis Specialist professionals meet the consumer at his or her location, stay to stabilize the situation and determine if further care is needed. This service is also available through the same delegate line as the JFK Mobile Emergency Team. Central Intake / Crisis Intervention provides a central access point to help reduce the use of psychiatric hospitalization and connect people to services with the least disruption possible to their everyday lives. Outreach to the homeless is ongoing and takes place in various areas of the city as the need arises.

Outreach program goes into the community and find the homeless in their environment, meet their basic needs, and build relationships with them and connecting them with other organizations in the community. Distributes food, water, clothing, blankets, and other services.

* Homeless Supportive Services: Provides intensive case management and referrals to other support services for single, adult, chronically homeless individuals. Assist street homeless men and women to connect to community resources as they strive to move from homelessness to self-sufficiency.
* Social Security Representative Payee Program: Provides financial monitoring and fund administration to individuals receiving social security who are unable to manage their own finances.

Connects and builds relationships with women and girls on the street, providing for their individual needs.

Serves monthly community meals. Provides volunteer opportunities in outreach to unhoused members of the community through visits and other activities.

Teams of volunteer health professionals, formerly homeless persons, and other outreach staff visit homeless individuals where they live â?? in camps along riverbanks, in alleyways, or beneath bridges and highway overpasses. Outreach teams provide medical care, basic supplies to provide relief from cold weather, and help individuals access housing and other social services.

An emergency street outreach program providing case management and social service navigation assistance to individuals experiencing homelessness. Conducting client-led case management allows clients to address concerns they may have and reach the assistance they may need/want.
