    Medical and Rehabilitation Care
HCR ManorCare provides a broad spectrum of services in communities across the United States. If you know the type of care you or your loved one needs, select from the list below.

    - Care in a Nursing and Rehabilitation Center: Our comprehensive medical and rehabilitation center serves the care needs of a wide variety of patients. Many are looking for active treatment to get back on their feet as they recover from surgery, a severe injury or a debilitating illness so that they can return home. Others can no longer be on their own and choose our center as their home.

    - Care at Home: We know that most people prefer receiving care in a familiar setting where they are surrounded by the love, patience and understanding of family and friends. Heartland Home Health Care has designed services to help patients strengthen and increase their ability to care for themselves in their homes. We believe that health and well-being starts with understanding that illnesses can impact loved ones and may also impact a patient's hopes and aspirations.

    - Hospice Care: As patients' health care needs change, the way in which we manage those needs also changes. This is especially true with an advanced illness. Our mission at Heartland Hospice is to support the patient and family wherever they are in their health care journey and to respect their goals and values.

    - Outpatient Physical Rehabilitation Care: Our licensed therapists, which provide care through Heartland Rehabilitation Services, provide comprehensive programs in sports medicine and industrial, pediatric, physical, occupational, speech and orthopedic therapy for patients recovering from major surgery, strokes, heart attacks, neurological and orthopedic conditions, and other illnesses, injuries and disabilities.

    - Alzheimer’s Care: We offer specialized Alzheimer's care for people with Alzheimer's disease and related memory impairment. Our specialized settings and services help us meet the very special needs of people in various stages of the disease. We work closely with family members to provide support and counseling.

    - Assisted Living: Assisted living is an appropriate option for seniors who want to maintain their independence and privacy but require some supervision and support. Our assisted living centers encourage socialization through a broad spectrum of activities.
