  • Supported Living - Achieva provides supports designed to meet individual and family goals to individuals with disabilities in their natural homes.

  • Lifesharing - Achieva works with families who desire a family living arrangement for their loved ones with disabilities if the individual chooses not to live with their natural family. Achieva locates and screens families who will share their homes with the individual.

  • Parenting Support - Achieva offers a parenting education program for parents and caregivers who have intellectual and developmental disabilities (IDD). With appropriate support, 80% of the children in these families will maintain equal footing with their classmates.

    The Delaware Valley Veterans Home was dedicated on November 1, 2002 and is the commonwealth's sixth and newest veterans home operated by the PA Department of Military and Veterans Affairs. Our facility serves 171 veterans and their eligible spouses in a caring and home-like setting. It is a spacious facility with skylights and large windows throughout the building.

    We are located just minutes from I-95 and the Pennsylvania Turnpike in Northeast Philadelphia at the border of Bucks County. The home offers a rural setting despite its city location with meadow and woodland views all around. Adding to the tranquil setting are the frequent sightings of deer, hawks and a variety of other wildlife.

    Our facility includes many amenities, such as, Canteen, a large Community Room for activities, Library, Barber Shop, Beautician and Chapel. We have two fulltime Physicians on staff and a team of RN's, LPN's and CNA's on each shift providing around the clock continuity of care for each resident. We are a smoke-free environment.

    The Southwestern Veterans Center provides a broad spectrum of health care and related services to veterans who are disabled, chronically ill, or in need of specialized care.

    The center serves 236 veterans and spouses; 160 beds with full nursing care, 44 beds for residents with Dementia/Alzheimer's disease, and 32 beds for veterans needing minimal care. Our focus is to assist veterans in functioning independently and at their optimal levels within the limitations of their illnesses.

    Our specialized services include a chapel, pharmacy, canteen, barber and beautician services, dining rooms, library, and a multi-purpose room for large gatherings. We also participate in the HealthNet System, which provides a telecommunications link between the center and medical experts throughout the state.
