Provides coordinated resources, case management and social work services to adults who are physically disabled in Chester County and parts of Bucks, Delaware, Lancaster, and Montgomery County. Other services provided include information and referral, home modifications
, adaptive equipment, transportation services, Community Service Programs for Persons with Physical Disabilities, personal emergency service systems for people with disabilities who qualify, and referrals to other services.
