Intake Procedure: Applications accepted through online submission. Call or stop in for information or assistance with filling out an application.
Intake Requirements: Names, dates of birth, and Social Security numbers for all household members. Information on the amount and type of income, assets, and expenses for each household member will be needed at time of application.
