Fee Amounts: Co-pays are on a sliding scale. Fees based on mileage. Call for more information
Intake Procedure: Call to schedule a trip and be prepared to provide the following:
  • full name
  • appointment day and time
  • destination arrival time with destination address and name
  • if a mobility device is being used
  • if an escort is coming
  • time of pickup
Intake Requirements: Must call to schedule ride no later than 2:00 pm one business day prior to the appointment. Trips may only be scheduled maximum of one week in advance. Same day service may be available depending on service levels.

Helpful Tips: Pets are not permitted; however, service animals are allowed as long as the call taker is notified of this
Facility/ADA Access: Vehicles are equipped with wheelchair lifts
