Springfield - Family and Community Service of Delaware County - HIV/AIDS Medical Case Management
The goal of Medical Case Management (MCM) is to empower persons who are living with HIV to maintain optimal physical and emotional health through ongoing assessment of needs, linkage to medical care, retention in medical care, treatment adherence, service planning and monitoring, coordination of services and advocacy.Our Medical Case Managers meet with their clients at least once a month to update goals and to determine progress. Case managers meet with clients in the office, in their homes, or any community setting that is convenient.
The following services are available for our MCM consumers: 340B Pharmaceutical Program, Transportation Assistance, Food Vouchers, Client Financial Fund, Support Group, and Peer Mentorship.
