Media Office - Nurse-Family Partnership
Provides an evidence-based program to support women having their first baby in which registered nurses conduct home visit throughout the pregnancy and continue until the child turns two years old.

Eligibility:
  • Women must be income - qualified (low - income)
    • Be having their first baby
    • Must enroll in the Nurse-Family Partnership program by the 28th week of pregnancy.
  • Hours: Monday through Friday, 9:00am to 5:00pm

    Intake Procedure: Referral may be made by another agency, provider, or client may make self-referral. Intakes are scheduled upon receipt and may be done by phone or in person, on site or in the home.
    Geographical Area: Chester City, Chester Township, Eddystone, Linwood, Marcus Hook, Parkside, Trainer, Upland, and Woodlyn
    Helpful Tips: Women are encouraged to enroll as early as possible in their pregnancy.
    Participants are eligible for referral to the Housing Opportunities Program for Equity (HOPE).
