Transitions Program
Provides bridge housing and support for up to one year while families work toward financial stability and permanent housing. This program is not a shelter.

Eligibility: Must meet all criteria:
A) Single mother with custody of one or more children
B) Literally Homeless or fleeing Domestic Violence
C) Income guidelines (at or below 200% federal income poverty guidelines)
D) Ability to function/live independently
E) Willing to work on self-sufficiency goals
Hours: Monday through Friday, 9:00am to 4:00pm; PA 2-1-1 hotline, 24/7

Fee Amounts: Families pay a monthly program fee based on their income as well as their electric bill for their unit
Intake Procedure: For new families, must go through the Dauphin County Coordinated Entry System - Call PA 2-1-1 by dialing 2-1-1 to complete the pre-screening for the Coordinated Entry System. Do not accept applications on-site.
Intake Requirements: Proof of homelessness and verification of current income status
