Central Office - Aging Waiver
The Aging Waiver Program offers eligible participants an alternative to institutional care. For those eligible clients/consumers, who have been assessed with a nursing home level of care and determined financially in need by the DPW/CAO, case management staff develops an individual service plan and arranges for a variety of in-home services to insure the participants can safely remain in a community setting. Benefit counseling is made available to those consumers that may experience difficulty in pursuing financial eligibility for medical assistance to qualify for this program.

Eligibility: Provides services to those persons 60 years of age or older who meet all of the following requirements:

Meet Medicaid financial requirement
, Require the level of care of a nursing home
, Choose community-based services rather than a nursing home
, Can be safely served in the community based on a care plan of services approved by the state.
