Erie - Chronic Disease Self-Management
Provides Chronic Disease Self-Management (CDSM) workshops. The CDSM workshops are an evidence based prevention and health promotion program that addresses common issues faced by people with chronic conditions. The program includes six weekly classes for 2.5 hours per week. The class sizes are small: 10-16 people. Content includes information on appropriate exercise, healthful eating, medications usage, improving communication with healthcare providers, problem solving skills, and more.
