Main Administrative Office - Blended Case Management Adult
Provides case managers who help individuals access and link to community services needed in order to live independently and maintain good mental and physical health. Case managers provide advocacy in service coordination and assist in areas such as but not limited to: housing, education/work, daily living, income/benefits, drug/alcohol services, and physical health care.

Eligibility: 18 years old and older, resident of Erie County, diagnosis of a serious mental illness such as schizophrenia or bipolar disorder, significant difficulty independently managing responsibilities at home, at work, or in the community, recent psychiatric hospitalization or frequent contacts with Crisis Services.
Hours: Administrative hours: Monday through Friday, 8:30am to 4:30pm
Program hours: By appointment, Monday through Sunday
