Assists with the transition from homeless through the PATH program. Provides rental assistance. Offers security deposit. Assists with a one-time back rent payment if no more than one month's rent is owed.

Eligibility: A) Diagnosed with serious mental illness and can provide documentation
B) Must have income that proves that the individual can sustain the desired living situation for at least 90 days
C) Is currently homeless or at imminent risk of homelessness

Intake Procedure: Dial 2-1-1: Individuals/families must have a referral sent by a housing specialist to receive these services. Once a referral is received, a call, within (3) business working days, will be placed directly to the individual to follow up on the current information and determine if the individual meets the eligibility for services. At that point, an intake appointment is scheduled with the individual. Individuals will need proof of income for the household, documentation of serious mental illness, amounts of monthly bills, and eviction notice or lease if seeking security deposit assistance.

Helpful Tips: Assistance is available until funds are depleted. Emergency supplies for individuals experiencing homelessness with no identified shelter include: tents, sleeping bags, flashlights, first aid supplies, camping utensils, hygiene items, backpacks, non-perishable food items, etc.
