Intake Procedure: To be eligible for Medical Assistance transportation you must have a valid Medical Assistance Access Card and fill out the MATP eligibility application.
Geographical Area: Bradford, Sullivan, Tioga
Helpful Tips: Must call before 11 am on the business day prior to appt. to schedule transportation. Must cancel transportation service at least 2 hours prior to arrival of driver.
