Intake Procedure: Application
Intake Requirements: Photocopy of any of the following for proof of age: Birth Certificate, Baptismal Certificate, Driver's License, Pace Card, State ID, Armed Forces Discharge Papers, Passport or Veterans Universal Access ID Card.
Geographical Area: Bradford, Sullivan, Tioga
Helpful Tips: Remember Senior door to door service is a shared ride program and not a taxi.You will be sharing the vehicle with other riders, so trips may take longer than in a private automobile and we may need you to adjust your appointment times to insure on-time arrival for everyone.
