Media - Psychiatric Rehabilitation Assessment
Psychiatric Rehabilitation Assessment is point of entry for people with serious mental illness to be admitted into any of the psychiatric rehabilitation services in Delaware County. The assessor makes referrals and ensures the connection with day rehabilitation, vocational/educational, Mobile Psychiatric Rehabilitation and clubhouse programs throughout the County.

Eligibility: CareLink currently provides services to residents of Delaware, Chester, and Montgomery Counties in Pennsylvania, Available services vary by county and individual CareLink site.

Program participants are typically referred to CareLink by county government agencies, social service providers, and health care providers.
Hours: Monday through Friday, 8:30am to 4:30pm

Fee Amounts: CareLink's services are primarily funded through the offices of behavior health in the counties they serve, but the Mobile Psychiatric services, Supported Living services, and Education and Employment support services are available with a private pay option to those living outside those counties.
