Full Care Residences help adults with mental illness learn the skills they need to live independently and become fully integrated into the community.


Includes: 24-hour staff support and supervision
. Assistance with medication
Help with meal prep, self-care, and health care coordination
. Social and community activities.
Coaching in constructive leisure time and using public transportation. Strategies for obtaining educational and vocational opportunities

Eligibility: CareLink currently provides services to residents of Delaware, Chester, and Montgomery Counties in Pennsylvania, Available services vary by county and individual CareLink site.

Program participants are typically referred to CareLink by county government agencies, social service providers, and health care providers.
Hours: Program is 24/7. Office hours are 8:30am to 4:30pm
