Early Intervention Program provides an array of home and site-based services for young children from birth to age five, who show delays in their physical, emotional, cognitive, or social development. A multidisciplinary evaluation identifies the child's need for services that may include special instruction; physical, occupational or speech therapy; behavior therapy; and nursing and nutrition consultations. EIP instructors coach parents in their home environment to interact with their child through play activities that foster the youngster's individuality and self-confidence and build developmental skills. Staff and specialists work toward a trusting partnership with families that allows each child to blossom.

Providing live-in home care means that a caregiver lives in your home, either longer-term or for a short period, to help with anything from spontaneous needs to planned outings. With overnight care, a caregiver comes by the house to provide a few hours of care before bed and in the morning, yet remains at the house throughout that period. In addition to providing homemaking and companionship services, overnight caregivers can provide several personal care services such as bathing, dressing, oral care, and restroom assistance.

Offer post-surgery recovery and rehabilitation care that will provide you the support you need to fully recover right where you desire. Caregivers can help with:

Operates an outpatient primary medical care clinic for Veterans through the Lebanon VA. Offers primary care and annual physicals by appointment, preventive services, women's health services, routine lab work, EKGs, immunizations, medication review, tele-mental health and social work services, treatment of conditions such as diabetes, hypertension, respiratory disease, common cold, influenza, tobacco cessation and specialty referrals.

Telehealth:
As a convenience to our patients, some services are offered by electronic means. Individual or group appointments may be performed via video to the Lebanon VAMC or other medical facility. Also, various tests can now be performed at any one of the six outpatient clinics, with the analysis being performed at the Lebanon VAMC or other medical facility.

Family Based Services provides 32 weeks of intensive community and home-based therapy and support for children and adolescents (up to age 21) with emotional and behavioral issues and their families. Children and adolescents using this program are typically at-risk for out-of-home placement so focus is placed on working with the family to successfully maintain the child in the home.

Early Intervention program is designed to help children between the ages of 3 and 5 overcome areas of delayed development. The program advocates on behalf of these youngsters, ensuring they receive appropriate special educational services. Children can receive speech therapy, occupational and physical therapy and other special instruction.

Home health care services are provided throughout the community to patients of all ages with acute and chronic illnesses. Nurses, physical therapists, occupational therapists, speech therapists, and home health aides promote recovery; restore health; teach patients and families how to manage chronic illness; and maximize independence in activities of daily living. Specialty services provided by expert clinicians address the specific needs of patients with Alzheimer's Disease, diabetes, heart disease, stroke, cancer, dementia, wounds, incontinence, ostomies, and mental/behavioral health problems.

Mental Illness provides services specifically designed for the seriously mentally ill. Many of these individuals have spent a significant amount of time in state institutions, struggle with drug and alcohol dependency, have multiple physical problems and have previously displayed violent behaviors. They have little or no social support, but seek new opportunities and better quality of life. Often, new residences act as the foundation from which a fresh, stable start can be built.

Provides nursing, physician, and social work services; physical, speech, and occupation therapy; assistance with personal care, meals, and light housework; nutrition counseling; spiritual counseling; oxygen, medical equipment and supplies; volunteer support services; medication to alleviate symptoms; bereavement support and grief counseling; short-term acute in-patient care;
and 24-hour at-home care during a crisis.

Offers a full cycle rehabilitation program, including physical therapy, occupational therapy, and speech pathology.

Children with hearing loss learn about themselves as they experience the ever-changing world around them. Our curriculum enhances children's listening, speech and language skills while fostering their social, emotional, cognitive, physical and creative development.
In addition to being in the classroom, children have speech language therapy every day which is facilitated by a speech and language pathologist or teacher of the deaf.
Daily curriculum includes math, art, reading, science, history, dramatic play, free choice, music, and sensory-motor integration which is all taught through a central theme, worked on both in the classroom and in therapy. Specialized activities address the individual needs of every child and family.

Provides an after-school program that emphasizes the value of human life. Program cohorts run 12 weeks. Activities include training in influencer leadership, communication, study skills, career development, mental and physical wellness, entrepreneurship, and blockchain basics. Also includes dog training and art therapy involving painting, audio and video production, and performance. Each session begins with a meal, whoosah, and a "how was your day" check-in.

Provides consultation services to teachers and other school-based personnel throughout Chester County. Students referred for OT consultation may be experiencing difficulty functioning in the classroom due to fine motor, sensory motor, handwriting and/or organization issues. Students referred for PT consultation may be exhibiting gross motor concerns.

Services:
Observe students and provide written reports including observations with specific strategies to enhance studentâs classroom performance.
Suggest and/or provide adapted learning materials when appropriate.
Provide on-site guided practice for pencil grasp, handwriting, scissors skills, computer ergonomics, organization and fine motor and movement activities.
Present workshops on topics such as handwriting, fine motor skills, sensory modulation and classroom ergonomics.
