    As part of the complete Continuum of Care, each patient is provided with a comprehensive aftercare plan. Transportation is offered at each of our locations, which provide the following services:

    Partial Hospitalization
    PHP is a step-down program between inpatient and outpatient. PHP allows more structure than outpatient and is less intensive than inpatient. It allows patients to have more support and structure while building a foundation for recovery. It is part of the continuum of care that Malvern Institute provides.
    - Intensive Outpatient - Day and evening sessions are offered at all of our locations to accommodate patients schedules.
    - General Outpatient - Typically meets twice weekly at each location, designed to offer continued support and education to patients.

    Each patient is assigned to a specific treatment plan based on their individual needs. The plans are tailored specifically to each patient and addressed in specialized, small group and individual sessions with their primary counselor, including:

    - Impulse Control - This is a common issue associated with addiction. Education is focused on learning how to change behaviors and to exert control over thoughts and impulses to assist with curbing addictive behavior.
    - Anger Management - For those patients who exhibit anger control issues, educate patients on emotions, specifically on anger, and ways to cope and deal with extreme emotions, such as anger.
    - Development of a sober structure - Many patients do not have structure in place in their daily lives. Structure is important in recovery and is incorporated into the program to help clients. This includes incorporating 12-Step meetings and outpatient sessions into daily schedules and employment.
    - Development of sober support systems - Many patients surround themselves with friends and family members that also drink or use drugs. Development of sober support occurs through incorporating 12-Step meetings and connections to other sober individuals.
    - Coping Mechanisms - Many patients do not have necessary coping skills to deal with common daily stressors such as anger, emotions, boredom and relationships. Coping skills include appropriate expression of emotions, asking for help when feeling overwhelmed, being honest with cravings and developing healthy actions when faced with unexpected obstacles.
    - Denial - Patients often don’t realize the extent of how addiction has impacted their lives and loved ones, employment, finances and health. Patients are taught to outline consequences of addiction and the negative impact drugs/alcohol has on their lives so they are able to understand importance of recovery.
    - Grief and Loss - Education focuses on the emotions that goes along with grief and ways to cope in a healthy manner.
    - Resistances to treatment - Many patients come into treatment because of pressure from family and friends and don’t utilize education and tools learned in treatment. These patients are given treatment plans that target this issue and education is provided on internal and external motivation on how this can impact their recovery.
    - Minimization of issues - Patients often minimize the impact of drugs and alcohol on their lives and don’t want to face the extent of the impact. This is a common defense mechanism. Education and coping skills are also taught on how to deal with this.
    - Peer Influence - Many young patients have a difficult time dealing with becoming sober at such a young age and are especially susceptible to peer influence. Education focuses on the seriousness of peer influence and the impact on relapse. Guidance is provided on how to develop new friends and support groups through young adult programs and 12-Step meetings.
    - Relationship Counseling - Relationships are often affected by patient’s drug/alcohol use and family sessions are offered to assist couples dealing with addiction. Also, there is a special track group that focuses on relationships and how to improve communication in relationships.
    - Aftercare Planning - All patients are given an aftercare plan upon discharge to ensure continuity of care. There is an aftercare specialist on staff to assist with long-term aftercare treatment or living arrangements such as halfway or recovery houses.
