Project ONE is a case management program for HIV-positive individuals in Chester County who need assistance in obtaining medical care, counseling, mental health treatment, substance abuse treatment, food and housing. It also provides prevention and education programs to high risk individuals and groups. All services are confidential and, when appropriate, may be provided in a client's home or on the phone.

Provides employment and education supports for adults working toward self-sufficiency. Participants receive one-on-one intensive case management, wrap-around supportive services, resources, and referrals. Works directly with participants to create a detailed goal plan, obtain a credit report, create a monthly budget to increase knowledge of money management. Assists with creating, updating, and maintaining a current professional resume to improve success in obtaining employment opportunities, finding adequate affordable housing options, and finding employment or better employment opportunities.

NSC's Health and Wellness Program helps refugees, immigrants, asylum seekers and victims of torture connect with needed case management, health care, mental health care, and wellness support. We offer an integrated model of care, matching clients with needed case management, health care, mental health care and wellness support.

Offers a day shelter that provides breakfast; mail, fax, and phone services; lockers (30 days max); public restrooms; hot showers with towels and soap provided; crisis intervention and mental health counseling; social service applications and referrals; a monthly legal aid clinic; and an ID clinic to obtain state IDs, birth certificates, and social security cards.

Behavioral Health Assesment Services offers an Acute Stabilization counseling providing brief, time-limited therapy and counseling to identified, at-risk clients. In addition, the Acute Stabilization Counselor works with existing clinical and case management staff to promote stabilization.

Assessment Include:

- Completion of intake packet

- Discussion of concerns for treatment

- Present all financial information needed to determine a reasonable monthly fee or other source of payment

Offers weekly home visits with a registered nurse who provides health and child development activities; prenatal education, support, and referrals; nutrition, mental health education, and support; substance abuse education and support; referrals and assistance in meeting personal life goals.

Offers therapy to men, women, and children of all ages for any situation. Specializes in sexual and domestic violence while working to end all types of violence. Sessions run for 30 minutes to 1 hour and can be done in a group or individual sessions. All services are confidential and will either meet at a safe space for the client or will be arranged in a secure location.

Provides treatment for addiction, diagnostic psychiatric evaluation, psychiatric medication managment, short-term counseling for depression, anxiety, and other behavioral health conditions, and referrals for more comprehensive and long-term counseling services.

Provides services to both children and adults with mental health and intellectual disabilities.
