Pennsylvania Prescription Program (65+)
PACE (Pharmaceutical Assistance Contract for the Elderly) and PACENET are prescription programs for Pennsylvania residents age 65 or older with limited income. You can apply for PACE/ PACENET if you have health insurance or limited prescription benefits through other insurance. You are NOT eligible for these programs if you are eligible for prescription benefits under the Medical Assistance program.

Medicare Part D Prescriptions
Under the APPRISE insurance counseling program, specially trained volunteers provide information regarding the numerous plans covered by Medicare and assistance with enrollment in the plan of your choice typically during Open Enrollment.

One-on-one counseling, holds seminars open to the public on topics such as Medicare Basics, programs to help save money and Medicare Prescription Coverage.

Assists with comparison and selection of Medicare supplemental Insurance Plans, Medicare managed Care Programs, Medicare prescription, long-term care insurance policies, and other health insurance and public benefit programs.
Provides claim filing and other related paperwork assistance, advocacy, and referral to other appropriate federal & state agencies are also available.

Provides a medicare counseling program designed to help older adults. The PA MEDI program is a State Health Insurance Program (SHIP) funded in whole or part by a grant through the Administration for Community Living.

Offers a collection of books, magazines, and audiobooks; summer programs; Internet access, and information on public assistance programs.

Offers a faith-based continuing care retirement community which provides all levels of care from independent living in townhomes or suites to assisted living, skilled nursing, short-term rehabilitation, and a specialized memory unit. Also offers Medicare counseling and enrollment services to potential or current residents.

Pennsylvania Medicare Education and Decision Insight, PA MEDI is a federally funded program administered through Area Agency on Aging and is designed specifically to help Pennsylvanian's on Medicare. Pennsylvania Medicare Education and Decision Insight, PA MEDI counselors answer questions about Medicare benefits and provide easy to understand information. Counselors are available to review and answer questions related to Medicare Advantage Plans, Medicare Supplement Plans, Medicaid, Medicare Appeals, financial assistance, and Medicare Prescription Drug Plans. Pennsylvania Medicare Education and Decision Insight, PA MEDI is a free unbiased resource for anyone who is Medicare eligible. One-on-one counseling sessions are held by certified volunteers committed to assisting beneficiaries with any Medicare questions or concerns. PA MEDI volunteers are also available for Medicare presentations and information sessions.

Cedarview Apartment complex is 200 one-bedroom apartments for low to moderate-income residents, 62 years of age or older, as well as income qualified persons younger than 62 who are on disability. * Information and Referral - answers to questions on the availability of services and programs for older adults.
* Legal Services - provided in non-fee generating cases via a contract with North Penn Legal Services, Inc.
* Nursing Home Transition - Information about availability of long term care in a variety of settings - in the home, or in assisted living, and planning/coordination in making a move to an alternate setting including arranging for necessary services, for current residents of nursing homes.
* Ombudsman - receives, investigates and assists in resolving complaints about quality of long term care. Provides information on long term care issues.
* Protective Services - activities, resources and supports to prevent or intervene in abuse, neglect, exploitation and abandonment cases.
* Specialized Transportation - limited availability of rides to medical appointments and social service agencies.
* Care Management - casework services for assessment and care planning, and authorization of the following:
* Adult Day Care - caregiving in an organized setting for less than a 24 hour day. Excellent for caregiver respite.
* Emergency Energy Assistance - bill payment or provision of fuel to avoid utility shut off or lack of fuel in crisis situations.
* Caregiver Support Program - educational services and financial assistance to family care providers.
* Home Delivered Meals - hot/cold meals delivered to maintain necessary nutrition.
* Home Health Care - skilled nursing, therapy and aide services.
* Nursing Home Respite Services - respite for overnight absences of caregivers.
* Personal Care - in-home help with bathing and other personal hygiene, laundry, shopping and meal preparation.
* Pre-Admission Assessment - level of care determination for those considering admission to a long term care facility.
* Waiver Services - Medicaid-funded in-home services of the types listed above, and more, to maintain nursing-home eligible older adults in their homes.
* Senior Farmer Market Nutrition Program - provides food vouchers for residents 60 years old who meet income guidelines
Information and referral service provides information on the availability of services and programs for older adults. Services may be delivered directly, purchased from a community agency, or available through a cooperative agreement.

Senior center offers various classes and activities. Must pay to play bingo or pool. Hot meal is served Monday through Friday, 11:15am, but must arrive by 10:45am. Individuals must call one day in advance for lunch reservation. Attendees are asked to make donations (what they can afford) for meals and transportation. Transportation is provided (must register with Easton Coach first).
