A pediatric extended care program for medically fragile children. Daily care by registered nurses incorporates therapeutic play with nursing care, individualized to meet the needs of each child; 1-3 ratio of staff to children.

Provides extended day care services for infants, toddlers, pre-schoolers, and school-age children located in the Belle Vernon area.

Provides a process where families have the opportunity to develop a plan that will ensure children are cared for and protected from future harm in ways befitting their culture and lifestyle. Extends on-going responsibility for child safety, well-being and permanence to families, communities and natural support systems. Families are directly involved in the planning and protection of their children.

Diakon's Family-Based Mental Health Services program provides counseling services to individuals and their families, with services primarily occurring in the home, although they can be extended to the school or community setting. The goal of the program is to provide families with the skills and resources necessary to prevent their child or adolescent, who is experiencing significant emotional problems, from requiring out-of-home placement.

Services include:

Home Health â provides services for homebound patients who require skilled care by a nurse, a physical therapist, or a speech therapist. Clinical programs include: Cardiac, Diabetes, Maternal-Child, Medical/Surgical, Oncology, Orthopedics, and Ostomy and Continence.

Hospice â provides palliative care for patients whose illness (such as cancer, advanced heart, lung, or kidney disease, Alzheimer's Disease, or AIDS ) no longer responds to medical treatment. Services include comprehensive medical, psychological, and spiritual care for the terminally ill, and support for the patientâs family.

Home Infusion Service â provides intravenous drug or nutritional therapies to patients in the comfort of their own homes. This enables patients to continue daily activities with minimal disruption. Infusion services include IV antibiotic therapy, TPN, chemotherapy, hydration, pain management, IVIG, steroid therapy, cardiac and transplant related therapies, and other IV therapies.

Extended Hom

Provides a substitute family life expierence for a child who needs care for a temporary or extended period. The primary goal is to reunite families or to ensure that the child will leave foster care for a permenant placement.

Foster parents play a significant role in assisting a child and birth family when the child cannot reside in his or her own home. The heartfelt services provided by a foster family in a warm, loving, home environment to address a child's emotional, behavioral, medical, social, and developmental needs can be instrumental in helping the child to heal and flourish. Children participating in Foster Family Care may be infants with special needs, toddlers, youngsters, or young adults up to 21 years of age.

Silver Springs d/b/a Gemma Services provides financial assistance, ongoing training and professional guidance and recognition for foster parents committed to meeting the medical, emotional, and behavioral needs of a child. Case management and outreach services are extended to each child's foster family and family of origin.

Camp Kidsâ Place provides child care for school-age children all summer long. Join us for an enriching summer of art, sports, dance, reading, outdoor fun and much more. Each week's activities include a trip to an outdoor pool and every session features a special trip. Camp Kids' Place is for children entering grades K-5. Camp is open from 8am to 6pm. Tuition for each two-week session is $425; we welcome children enrolled in subsidized care. The Extended Assessment Program offers up to 24 hours of assessment and case management to children in Delaware County. The program links children with the services that will best aid their overall growth and development. A fully-credentialed Master's -level clinician and a clinical case manager oversee each child's case and collaborate with the school, community, and home to determine the most suitable resources for the child. Recommendations may include continued assessment, EPSDT/Wraparound evaluation, or another level of care.

A before and after school program is provided to Wissahickon students through the Ambler Area YMCA School Age Program. The YMCA is committed to providing a quality program that is both fun and safe for children.

Services for families that are required to travel far and spend several extended time away from home in order to receive medical care for their ill child at Janet Weis Children's Hospital. Services meet familiesâ day-to-day needs so they can remain close to their child while under medical care. Overnight lodging is also available at the Ronald McDonald House to provide relief of the financial burden on budgets already strained by medical expenses. The lodging includes guest rooms with private bathrooms, efficiency rooms with kitchenettes and private bathrooms for families with children diagnosed
with cancer and in outpatient treatment, a kitchen stocked with staple products, a laundry room stocked with laundry products, a living room, a private sunroom, an interior play room, an exterior playground, public TV rooms,a front exterior patio and public Wi-Fi.

Prescribed Pediatric Extended Care Center, is licensed by the Pennsylvania Department of Health. The Trinity Child Care Center was specifically initiated for children who are medically fragile or technology dependent. This service can be utilized as an alternative or a supplement to in-home nursing care

Child care services for children ages 13 months to 12 years.
