Avenues - Home & Community Program

Designed to promote independence to all the individuals that it services. An opportunity for individuals with intellectual disabilities to become an integrated and active member community in which they reside. Habilitation services may be used in conjunction with other hourly services as long as they do not overlap and depending upon an individual’s available funding. This is a direct service provided in home and community settings to assist individuals in acquiring, maintaining, and improving upon their set goals. This is an outcome based service. All goals will be in the individuals ISP and Habilitation will be tracking the progress of those goals. Services consist of support in the general areas of communication, fine and gross motor skills, mobility, personal skills, appropriate social skills, relationship development, creating natural supports, and use of community resources. Designed to support individuals with intellectual disabilities by providing an hourly service to integrate the individual into their community and work toward accomplishing a set goal. Sample Goal Based Activities -Shopping at the Mall -Attending a Sporting Event -Bowling -Going out to Eat -Going to Public Library -Exercising -Attending a Craft Class -Going to Local State Parks -Going to a Block Party -Attending a Community Dance An Example Scenario If the individual’s goal is to learn to budget money, an activity would be to window shop for an item at the mall. If they have the funds, assist them in purchasing the item. If they do not, explain how to save until they have enough to buy it at a later date.

PossAbilities provides a variety of services based on an individual's specific needs and abilities. Functional Skills Training Each program participant who attends the adult training facility (ATF) has goals to target specific areas of training, which are developed by the interdisciplinary team. These goals aid individuals in achieving their highest level of independence and environmental control. Training focuses on the following areas, based on an individual's needs and preferences. Some areas of training may overlap to increase independence in all aspects of their lives. Pet therapy Sensory Stimulation Fine motor skills Money management Household skills Community awareness and integration Socialization Recreation/leisure Activities of daily living Hygiene/grooming skills Street and safety awareness Arts and crafts Computer use Academics Music therapy/Community Connections Choir Exercise/healthy living Communication Community Integration Community integration is accomplished in conjunction with functional skills training. Program participants may learn shopping skills or daily living skills in a practical setting through weekly trips to local stores within the immediate community. Inclusion is promoted through volunteer efforts, including delivering Meals-on-Wheels, volunteering at local animal shelters, or cleaning up local parks and playgrounds. These activities are designed to give the community a chance to meet and interact with the ATF program participants. Community Supports Option PossAbilities Central also provides entirely community based programing to those seeing a non-traditional setting. The CSO staff to consumer ratio is 1:3. The individuals participating in this program do not attend a facility based program. Their activities are carried out in the community 5 days a week. This program focuses on volunteering and practicing community related skills.

Early Intervention (EI) services are specialized services provided to families in York and Adams Counties with young children (ages birth to three years) who have or are at risk for developmental delay. Early Intervention Services are completely free for eligible young children and their families. This program, provided with funding from the York/Adams Mental Health/Mental Retardation Program, is designed to provide family training and therapeutic services to enhance the child's development according to the family's priorities. The Community Supports Program (CSP) provides an array of individualized services and supports to children and adults with mental retardation. The CSP program provides support and assistance to individuals who are financially responsible for renting or owning their own home. The focus is on assistance with independent living skills such as budgeting, shopping, and medical appointments. They also provide services and supports for individuals who reside in the homes in which they grew up, their natural homes. The focus includes activities of daily living, community integration and socialization, as well as providing support and relief to the primary care giver. Still another component is component is providing services and supports to individuals residing in personal care homes and nursing homes. The focus is to provide socialization and extra support to the individuals in accessing their communities.

We help qualified and motivated individuals with disabilities find jobs that fit their interests and needs. We work with them to learn the required job skills. Our Employment Training Specialists provide on-the-job training while helping make necessary accommodations for each individual. We also partner with local school districts to teach job skills and employment-related social skills to help students transition from school to work.

    Since it was founded in 1962, Camp Pennwood has provided children with intellectual disabilities the opportunity to enjoy a traditional day camp. Our campers, ages 6 to 21, enjoy age-appropriate activities such as arts and crafts, archery, swimming, horseback riding and musical activities during camp's six-week summer run. With our dedicated staff and volunteers, approximately 80 campers benefit from a safe and educational rural camp experience each year P.A.V.E. Program: Each June, adults with intellectual disabilities come to Camp Pennwood to begin a week-long social and recreational experience highlighted by day trips and opportunities to enjoy nature.

    The John E. Walson Center at Via provides educational, social, cultural and wellness activities for adults with disabilities. Participants in this adult training program spend a portion of their time at our facility in Bethlehem, combined with opportunities to explore the community. The John E. Walson Center at Via introduces individuals to recreational activities and cultural events with the assistance of Via Community Inclusion Specialists. In addition, activities like cooking, nutrition, physical exercise, and cognitive skills development are integrated into our daily schedule to promote wellness and encourage social interaction. The John E. Walson Center at Via is a place for people to engage as independently as possible in the activities and experiences offered so they may be included in their communities and peer groups. The John E. Walson Center at Via is an Adult Training Service licensed by the Commonwealth of Pennsylvania's Department of Public Welfare. Facilities maintain compliance with Title 55 PA Code, Chapter 2380 Regulations for Adult Training Services.
