Avenues - Home & Community Program

Designed to promote independence to all the individuals that it services. An opportunity for individuals with intellectual disabilities to become an integrated and active member community in which they reside.

Habilitation services may be used in conjunction with other hourly services as long as they do not overlap and depending upon an individual’s available funding.

This is a direct service provided in home and community settings to assist individuals in acquiring, maintaining, and improving upon their set goals.

This is an outcome based service. All goals will be in the individuals ISP and Habilitation will be tracking the progress of those goals.

Services consist of support in the general areas of communication, fine and gross motor skills, mobility, personal skills, appropriate social skills, relationship development, creating natural supports, and use of community resources.

Designed to support individuals with intellectual disabilities by providing an hourly service to integrate the individual into their community and work toward accomplishing a set goal.

An Example Scenario

PossAbilities Central also provides entirely community based programing to those seeing a non-traditional setting. The CSO staff to consumer ratio is 1:3. The individuals participating in this program do not attend a facility based program. Their activities are carried out in the community 5 days a week. This program focuses on volunteering and practicing community related skills.

We help people with disabilities maximize their successes in the world of work. Based in the Downingtown Business Center, Brian's House Enterprises (BHE) provides employment training to adults with mental retardation, autism spectrum disorders, brain injury, and other disabilities. Individuals served may be incontinent or require mobility assistance.

BHE services include:
- comprehensive vocational assessment;
- supportive, competency-based pre-vocational, vocational, and psychosocial skill development;
- supervised workshop employment;
- community-based supported employment.

Based upon the needs and choices of an individual, BHE services may include job development, job skill training, workshop or community job placement, and extensive work adjustment coaching. We provide nursing coverage at our workshop, fully accessible facilities, implementation of services recommended by OT's and PT's, and positive approach behavior management procedures.

As with all Brian's House, Inc. programs, active supervision is coupled with an individualized approach to habilitation that includes ongoing consultation with family members.

Our primary goal is to help individuals with blindness and visual impairment to live safely, comfortably and efficiently within their homes. Instructors teach techniques for cooking safely, identifying canned goods, freezer items and medicines, managing laundry and housekeeping tasks, and keeping the household well-organized. Staff assists with marking appliances and with evaluating and implementing safety related changes within the home.

Instructors also introduce simple methods for dealing with everyday tasks such as money identification, telling time, and using the telephone. Clients learn new ways to communicate using large print materials, adaptive writing aids, and audio technology. When appropriate, basic Braille may be introduced.

Fundamental orientation and mobility skills and white cane techniques are oftentimes taught so that clients can travel safely in their environment.

Caring is key. Instructors provide adaptive ways to deal with recreational and social challenges that sometimes arise from vision loss. They also help clients to develop realistic and positive attitudes towards living a rewarding life.

Handi-Crafters
Handi-Crafters - After Work Program
Each Thursday after work, about 50 of the clients from our day program gather in one of two groups for fun and learning. The groups are led by a team of volunteers. Some of our volunteers are Handi-Crafters' staff who put on different hats for the afternoon, while others are from the community at-large. Each group has five or six "helpers" as they work through the afternoon's activities. These activities include musical sing alongs; exercising to "Chicken Fat", discovering their artistic talents, playing BINGO and many others. Special events are planned around the holidays.

Hope Enterprises, Inc. staff goes to the family’s/individual’s home to work with them. Some staff take the individual out to do activities such as shopping, through which they learn purchasing and money management skills; or recreational activities, such as going to a carnival, or to exercise at the local YMCA. Staff teach cooking and laundry skills, writing, personal hygiene, and decision making. They may take the individual to doctor’s appointments to help with understanding medical issues. In Home staff will follow up with therapeutic exercises that have been recommended by the physician, physical therapist, or occupational therapist.

The Community Supports Program (CSP) provides an array of individualized services and supports to children and adults with mental retardation. Individuals learn, maintain, or improve skills through their participation in a variety of everyday activities. They learn and use skills in the context of these activities which are necessary to live in the community, to live more independently, or to be more productive and participatory in community life.
The CSP program provides support and assistance to individuals who are financially responsible for renting or owning their own home. The focus is on assistance with independent living skills such as budgeting, shopping, and medical appointments.
They also provide services and supports for individuals who reside in the homes in which they grew up, their natural homes. The focus includes activities of daily living, community integration and socialization, as well as providing support and relief to the primary care giver.
Still another component is component is providing services and supports to individuals residing in personal care homes and nursing homes. The focus is to provide socialization and extra support to the individuals in accessing their communities.

    The residential program at Royer-Greaves School for Blind is licensed by the Department of Public Welfare 6400 regulations, as a Community Home for the Mentally Retarded. Residential program is centered around the Residence Hall, Activities Building which houses the gymnasium and indoor swimming pool, the School Grounds and the community. Each Individual is encouraged to attend their annual Multi-Disciplinary Team meeting to discuss their needs, wants and programs of attendance. An individual Program Plan(IPP) is then developed based upon the team meeting's findings which specifies the Individual's residential goals and objectives. Goals are designed to help individuals gain maximum independence. The program supports these goals by providing a continuous twenty four hour supportive, consistent and caring environment.
    Three Primary Phases of Focus

    Activities of Daily Living include those life skills and self help skills which an individual may need to address for independence. Self preservation skills and orientation and mobility are also addressed.

    Recreation and Socialization Skills focus on developing recreational interests and learning to interact with others through small and large group activities. Individuals are afforded the opportunity to choose activities of interest and build upon those interests for leisure time pursuits. Activities range from music appreciation, arts and crafts, the horticultural program, aquatic and modified athletics to reading and specialized interests.

    Community Life includes programs designed for each individual to learn and participate in community activities, which enhances their lives on an ongoing basis. Individual and group volunteers from the community participate in this program. The 'Windows to the World' program enables individuals to gain access to the community on a regular basis, whether it's to the post office, library or the mall. Activities also include dining out and trips to areas of interest such as the beach.
    Individuals are afforded the opportunity to become members of Explorer Post #103, regardless of age or sex. They participate in scouting activities as a post and also with other local Scout troops. The residential program is administered by qualified Residential Care Counselors and supervised by the Residential Program Director. Nursing services are provided twenty-four hours per day.

Our Employment Training Specialists provide on-the-job training while helping make necessary accommodations for each individual. Our campers, ages 6 to 21, enjoy age-appropriate activities such as arts and crafts, archery, swimming, horseback riding and musical activities during camp's six-week summer run. With our dedicated staff and volunteers, approximately 80 campers benefit from a safe and educational rural camp experience each year

    P.A.V.E. Program: Each June, adults with intellectual disabilities come to Camp Pennwood to begin a week-long social and recreational experience highlighted by day trips and opportunities to enjoy nature.
