  • Nursing Home Assessment Any person age 18 and older who desires placement in a nursing facility, personal care home or domiciliary care home can be assessed by the Care Management staff of the Adams County Office for Aging, Inc.
    • Domiciliary Care (Dom Care)Provides adults 18 years or older, who cannot live independently, with sheltered living in private homes with a family-like atmosphere.
    • Personal Care HomesSometimes called assisted living facilities, offer room and board and assistance with daily living activities.
    • Adult Day ServicesProvides functionally disabled older person with an array of activities in a protected environment which will enable them to maintain independence within the community.

    The Area Agency on Aging (AAA) provides for the physical, social, & emotional needs of the senior citizens of Erie County through informal and formal supports that enable seniors to make informed choices, to be safe, to remain independent, and to be involved in the community. A small co-payment is required for some services. These services, provided through the mandates of the Older American's Act, are designed to respond to those seniors remaining active and involved in their community, being cared for in their home where they feel comfortable, requiring placement services, and requiring protection and advocacy. Care Management provides an in-depth evaluation of client needs and the facilitation of the most appropriate services and monitors those services and the person's individual service. Services Include: Home Support - assistance with laundry, light housekeeping, meal preparation, and grocery shopping. Meals on Wheels - provides hot noontime meals to predominately homebound older adults delivered by volunteers. Sack meals are available for evenings and weekends determined by the service plan. Adult Day Care - subsidized day care which provides a structured, secure environment with daily activities and programs. Personal Assistance Services - provides or assists an individual with such needs as bathing (tub, bed bath, shower), washing hair, grooming, shaving, nail care, foot care, dressing, skin care, mouth care, and oral hygiene through a consumer directed approach. Home Support - assistance with medicine, personal hygiene, nail care, hairdressing, picking out coordinated outfits, help with getting dressed or applying makeup. Home Health - registered nursing services not covered by Medicare may be covered in the home short term and for medication distribution. Medical Supplies - assistance with medical supply needs.

Most services available to persons ages 60 and over, except for the 55+ Job Bank, Job Bank Workshop and Senior Community Services Employment Program which begins at age 55 and older. Domiciliary Care, long term Care Assessments and Caregiver Support Program available for persons 18 years and older. Services for 60 years and older: -Older Adult Daily Living Centers provided for persons with functional disabilities. Also provides daytime respite care to caregivers. Nursing services and mid-day meal are provided. Contracts with Landis Homes, Lancaster Generations, Masonic Homes, Moravian Center and Garden Spot Village to provide these services. -Home Delivered Meals - Work in cooperation with Meals on Wheels and other sources for homebound elderly persons -Emergency Fuel Assistance is provided as a last resort after a caseworker has assessed all other options. -Respite Care gives caregiver relief -Information & Referral assists persons to identify their needs, link them to appropriate services, and to share up-to-date information on issues affecting older persons. The Transportation Specialist, is also part of the I & R unit -Lawyer Referral Program for the Elderly: Clients are referred to participating attorneys who will offer advice on a broad range of legal issues. Initial 1/2 hour consultation is $5; Fees need to be discussed with the attorney for consultation over a half hour -Ombudsman Program investigates patient care concerns about nursing homes, personal care homes, and home health agencies -Protective Services: Intensive casework for persons who are abused, exploited, neglected, abandoned or who are unable to carry out activities of daily living due to mental or physical impairment, which is placing them at risk of imminent harm - Property Tax and Rent Rebate forms are available with assistance to complete them also available - Senior Centers offer noon meals and a variety of activities in the following county areas: Columbia, Elizabethtown, Lancaster, Lititz, Millersville, Quarryville and Reinholds - Flu Shots are usually offered in the Fall at the Senior Centers - Transportation to medical appointments and senior centers using Red Rose Access for those who are eligible - Offers Health and Wellness Programs to promote healthier lifestyles Programs for persons ages 18 and over: - Domiciliary Care offers protected community living arrangements for persons needing personal care and ongoing supervision. - Long Term Care Unit provides assessments to determine the most appropriate level of care for those needing financial aid for nursing, personal care, and domiciliary care homes. Intensive in-home services available for eligible persons who are at a nursing home level of care and have sufficient informal support. - Caregiver Support Program - Casework services and financial assistance with caregiving expenses for persons caring for individuals who are over 60 and are functionally dependent (requiring constant supervision) or between the ages of 18-59 with chronic dementia. Also offers assistance for grandparents who are the primary caregivers for grandchildren under the age of 18.     The Northumberland County Area Agency on Aging can assess individuals for: Geisinger Life Program - Living Independently for Elders A medical model Adult Day Care for individuals deemed Nursing Facility Clinically Eligible. It includes a comprehensive array of services such as primary care physician, speech, occupational, physical therapy and social. OBRA Everyone seeking admission to a nursing facility must first have an initial screening for mental illness, mental retardation, or other related conditions regardless of payment source. This screening helps determine whether a nursing home is appropriate and if specialized services are needed to treat those conditions. PDA Waiver The Waiver Program is designed to help people remain at home after they are both medically and financially approved for Medical Assistance. Home and Community-based long term care services can be provided as an alternative to nursing home care through the Waiver Program. Pre-admission Applicants seeking admission to a nursing home, personal care boarding home, Geisinger/Life or Waiver Program and applying for Medical Assistance must be assessed by the AAA to determine eligibility.
