-Nursing Home Assessment: Any person age 18 and older who desires placement in a nursing facility, personal care home, or domiciliary care home can be assessed by the Care Management staff of the Adams County Office for Aging, Inc.
-Domiciliary Care (Dom Care): Provides adults 18 years or older, who cannot live independently, with sheltered living in private homes with a family-like atmosphere.
-Personal Care Homes: Sometimes called assisted living facilities, offer room and board and assistance with daily living activities.
-Adult Day Services: Provides functionally disabled older persons with an array of activities in a protected environment which will enable them to maintain independence within the community.

Provides non-clinical primary mental health services to insure that the most seriously mentally ill individuals receive additional support geared to improving the quality of their lives and reducing psychiatric hospitalization. Casemangers use natural occurring supports in the community, i.e., family, friends and neighbors. Under the direction of a Rehabilitation Manager, our expert rehabilitation team includes Licensed Physical Therapists, Occupational Therapists and Speech Therapists. Services include psychiatric evaluation, medication monitoring, service Coordination, psychiatrist supports, crisis intervention, social skills training, psychiatric rehabilitation, vocational assessment, housing supports, activities of daily living, peer support services, and leisure time training.

The FACT Team is specifically designed for those clients meeting the several criteria for ACT but have a forensic criminal justice history. Services include psychiatric evaluation, medication monitoring, service coordination,
psychiatrist supports, crisis intervention, social skills training, psychiatric rehabilitation, vocational assessment, housing supports, activities of daily living, peer support services, leisure time training, service coordination, crisis assessment, intervention
symptom assessment/management, medication administration/monitoring, and Other Major Supports.

Provides support and services to enable older adults to continue to live in their homes and communities. Services may include Care Management, Assessment, Protective Services, Information and Assistance, PA MEDI (Insurance Counseling), Health and Wellness Programs, adult day services, in home meals, personal emergency response system, personal care, transportation, and caregiver support. The agency also has 5 Senior Centers throughout the County in which participants can enjoy a Congregate Meal and Programming.

Provides home health services to assist persons with disabilities and older adults with activities of daily living (ADL) to promote independent living;
Services include:

• Homemaker services such as shopping, laundry, light housekeeping, and meal preparation
• Fiscal management
• Personal care Assistance with self-administered medications, feeding, bathing, grooming, assistance with ambulation, toileting, and transfer activities
Attendant care for eligible adults under the age of 60, who are mentally alert and physically disabled, helping them live as independently as possible in their own homes while they seek or maintain employment
• Respite care
• Service coordination
• Activities of daily living

Haven Skilled Rehabilitation & Nursing (formerly ECU) provides comprehensive post-hospital medical and rehabilitative care for patients who have experienced injury or severe illness. Rehabilitation services help patients recover or increase their ability to perform daily activities. The rehab team includes physical and occupational therapists and a speech-language pathologist. Nursing services provide comprehensive resident assessments, development of individualized resident care plans, focused on the residents' personal strengths and goals. Skilled nursing provides services in the areas of wound care, IV therapy, assessment and patient/family education. They also provide residential care to those who are no longer able to live on their own or need care beyond that which family members can provide.
